Our commitment to your privacy

Elena Corral, LISW-CP is required by law to maintain the privacy of your protected health information (PHI), provide you with this Notice of our legal duties and privacy practices, and abide by the terms of the Notice currently in effect.

How we may use and disclose your information

We may use and disclose your PHI, without your written authorization, for treatment, payment, and health-care operations, as well as for certain disclosures required or permitted by law (for example, mandated reporting of suspected abuse, court orders, or serious threats to health and safety). All other uses and disclosures require your written authorization, which you may revoke at any time in writing.

Your rights

  • Request a copy of your PHI.
  • Request an amendment to your PHI.
  • Request an accounting of certain disclosures.
  • Request confidential communications by alternative means.
  • Request restrictions on certain uses and disclosures.
  • Receive a paper copy of this Notice on request.
  • File a complaint if you believe your privacy rights have been violated.
